Root Cause Analysis Explained ..


Well the whole process of this is quite step by step ,
Step 1. Problem Identification
The step 1 deals with identification and  clear cut idea of what the problem is whose cause has to be find out.Once this is clear the process goes to second step.
Step 2. Brainstorming of the various causes 
Now as i told earlier there are causes for this and this step deals with writing down all the causes which could be the possible cause for the main problem .
Step 3.  Classification Phase
Now as we know the various causes are from the five categories like People , Environment, Machines, Methods .
so the aim to classify and put the causes into there correct category.
Step 4 . Fish Bone Diagram
As the name suggests , fish bone diagram we need to draw the diagram in  the shape of a fish structure where the head of the fish is the problem and the fins and the branches  are the causes pointing to the main center line .
Step 5.  Analysis ( Root Cause)
We now need to focus on finding out what the main cause of the problem is ?
For this we see carefully as what are the causes which are in large numbers , the cause which is appearing the maximum times is the ROOT CAUSE.

Quality Management Tools , 1. Cause and Effect Diagram

Cause and Effect diagram is also called as Fish Bone Diagram or Ishikawa Diagram or Root Cause Analysis Diagram . It was invented by Quality Pioneer Kaoru Ishikawa  in late 1960's which proved to be a very crucial method for finding out the root cause of any problem.

The aim of the Diagram is to find out the major or most important cause behind a certain problem which is shown in the form of the head of a fish and the bones are the causes in this.

The idea is that the various causes for the problem are categorized into :
  • People
  • Material
  • Environment
  • Machines
  • Methods
These categories have further sub causes and futher sub-sub causes , so the main idea is to find out the root cause ( major) .
